Wrong procedure. Wrong body part. Wrong patient. Unintentionally retained objects. This category of extreme surgical errors are colloquially called "never events." Yet around 200 of them are reported in Texas each year.

- Mindi Crapo lost her 25-year-old daughter, Atianna Washington, to a fatal car crash in March 2025. The driver was not arrested that night. She was struck by an off-duty Dallas Police Department officer and then-president of the city’s largest police union, Jaime Castro.
An internal police investigation later found that DPD officers gave Castro preferential treatment and failed to properly investigate the crash, and Castro pleaded guilty to lying to police. Now, Crapo is leaning on a wrongful-death lawsuit to hold Castro and other officers accountable.

- Dallas police shot at an armed suspect on Wednesday afternoon near Mustang Inn in east Oak Cliff. A police spokesman said the suspect refused to drop a long rifle. After an officer fired a shot, the suspect fired at himself.
Police said they used Flock cameras to find the suspect, who had multiple felony warrants involving narcotics and a probation violation. The suspect’s condition was not immediately known and no officers were injured.
